Student Athlete Class Absence Form

This form is used to inform your professors of your known absences from class due to participation in competitions for the semester. It should not be used for absences due to illness or unexpected competitions due to rescheduling or other issues.

Instructions for Completing the Form

STEP 1: Make sure your student information is correct, then select your sport.

STEP 2: Complete the Class Information and Anticipated Absences sections. 

STEP 3: You MUST add your instructor's email to the Additional Confirmation Email Address field in order to send this form to your instructor.

• Your instructor's email address can be found on the syllabus, or you can use the "Find Your Instructor" tool.

NOTE: There might be several instructors with the same name who appear in the tool. If you are unsure which name to choose, you should refer to the course syllabus to get the correct email address.
