STEP 1: Make sure your student information is correct, then select your sport.
STEP 2: Complete the Class Information and Anticipated Absences sections.
STEP 3: You MUST add your instructor's email to the Additional Confirmation Email Address field in order to send this form to your instructor.
NOTE: There might be several instructors with the same name who appear in the tool. If you are unsure which name to choose, you should refer to the course syllabus to get the correct email address.
